We keep getting this error message randomly on all kinds of WMI sensors and Hyper-V sensors.

"The Windows on your probe system is too old. Please update to at least Windows Server 2008 or Windows Vista(we strongly recommend Windows Server 2008 R2 or Windows 7). (Performance Counter error 0xB0000005)"

We currently have PRTG on server 2003. Does this really mean we need to switch it to a more up to date OS, or is it a sign of something else? A lot of the sensors just go to warnings, then come back and the alarms never last long.

Performance Counters are not available when using a 2003 machine for PRTG to run on or if you are trying to monitor a 2003 server from a 2008. You can change these sensors to rely solely on WMI or you can upgrade the system to 2008 R2 to solve this issue.

The reason that you are seeing the alarms and they are going away is that these sensors are using WMI unless WMI is unreachable then it will fall back on Performance Counters. When this happens and they are not available, you see this message then it scans with WMI and the sensor comes back up.
