So, the Name field is optional. The Server field is where you would enter your server's IP address or domain. The Username and Password fields are your PRTG login fields.
For your protocol, you're gonna have to look at your server to determine if you have HTTPS set up or not.
Afterwards, the Start Screen is what you will launch into when you open the app.
