Sensor - Instances not found

I have four sensors that are giving the same exact alert. What do i need to do to rectify this?

Physical Disk: Percent Disk Time 3
Physical Disk: Disk Writes/sec 3
Physical Disk: Disk Reads/sec 3
Physical Disk: Current Disk Queue Length 3

Has the disk setup of the target host changed by any chance? Is this disk still there when recreating the sensors?
