i want to customize my Prtg Login page (index.htm) file by adding & play a video type webm using this page.

Thank you very much for your KB Post. Please be aware that we do not officially support customizations on the PRTG Webinterface. Do this at your "own risk", it may work, or it may not work. We cannot provide support for customizations though.
